This page is designed for prospective members who are interested in joining the appropriate forms and information to become a member of CNU's SCUBA DIVING CLUB!

Meetings at 8pm in Monroe Room                         on the 2nd floor of of DSU

 

Joining Criteria

This is not a complex or difficult process while it may seem like a lot. Please, just print out the following forms, fill them out LEGIBLY, and bring them to an officer or a weekly meeting.

Below is the list of forms:

- General Member Info Form.........Link

- Emergency Info/Contact (Club).....Link

- Emergency Waiver (CNU)...........Link

- Drug & Alcohol Policy................Link

- Diver Experience Level System.....Link

- Photo Copies of All Certification Cards (both sides of cert cards please, being a SCUBA Certified is not a requisite of membership)

- $10 for Dues

*Note: if you are under 18, you must have a parent/legal guardian sign with you.
